Crash Diets Might Cure Type 2 Diabetes

Dear Reader,

There have been reports that a so called "crash" diet might work for type 2 diabetes, a lone, rather small study has a 600 calories a day plan that cuts fat in the pancreas and urges insulin cells to wake up in only eight weeks. While the results sound promising, you need to understand this was an early study in just 11 obese patients who had type 2 diabetes. Calling the eating plan used in the research a "cure" is simply bad journalism. We can draw very limited conclusions from the findings.

If you are a diabetic, keep following the diet guidelines giving by your own healthcare team. Those who took part in this work all had close medical supervision - no one is suggesting you try this diet on your own.

Type 2 diabetes happens when not enough insulin is produced for the body to work properly or when the body cells don't react to insulin. What insulin does is help the body use glucose (sugar) for energy. As the demand increases on the pancreas to produce more insulin, there's further decline in function of the beta cells, known to produce insulin. Long-term diabetics have to inject insulin as their condition gets worse.

The team from the Human Nutrition Research Centre out of Newcastle University conducted the work, with funding supplied by Diabetes UK. Researchers wanted to find out if diet could reverse the resistance of the body cells to the action of insulin, and also stop the decline in those beta cells.

The research, appearing in the peer-reviewed journal Diabetologia, examined the theory that a severe restriction of the amount of energy in a diet can reverse the body's resistance to insulin. The work was a non-randomized trial with 11 patients who had type 2 diabetes for at least four years, and 9 people without diabetes who provided one-time measurements. The average age was 49.5 and the average BMI was 33.6.

Lab work was used to look at insulin sensitivity of the liver and other tissues of the body at the beginning of the work. A special MRI scan was used to look at the fatty acid content f the liver and pancreas. The subjects then began a diet made up of nutrient drinks that gave them 510 calories a day. This was supplemented by 3 portions of non-starchy veggies to give another 600 calories a day. More measurements were taken at one, four and at the eight week mark of the study. MRI scans were done again at 12 weeks, after the subjects had gone back to their regular diet.

After one week of the diet, fasting plasma glucose normalized. The production of sugar from the liver also went down, and the liver's sensitivity to insulin got better (from 43% to 74% after one week). By the end of the study, the fatty acid content of t he liver had gone down from 12.8% at the study's start to 2.9% at the end. The sensitivity of pancreatic cells to sugar gets better over the study period.

There was no change in insulin sensitivity of other tissues of the body other than the liver.

The team says that earlier work has shown weight loss surgery can reverse diabetes, and this supplied the notion that a sudden negative energy balance could have an effect on metabolism. Excess amounts of fatty acids are also thought to inhibit the workings of beta cells, so it was expected that a drop in fatty acid levels would improve the workings of these cells.

The calorie restriction plan also appears to stop the progressive decline in the workings of pancreatic cells that are responsible for producing insulin. Still, more well constructed studies with larger populations will need to be done.
